Intake Coordinator/Scheduler (Home Health) Faith Medical Services 1590 Phoenix Boulevard, Atlanta, GA 30349
Position Summary:
We are seeking an experienced and highly organized Intake Coordinator with a strong background in Home Health. This role is responsible for evaluating, monitoring, and managing the intake and referral process. The ideal candidate will display sound judgment while verifying, entering, and updating patient information and coordinating scheduling activities to ensure quality client care.
Qualifications:
Proven experience in the home health field (required). Strong understanding of intake and referral workflows in healthcare. Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and QuickBooks. Technical competency with scheduling and EMR systems.
Key Responsibilities:
Respond to all calls and emails promptly and in accordance with company policies. Enter all intake information accurately into company software. Complete and route intake forms for admission approval. Manage referrals for skilled and unskilled services from hospitals, insurance companies, and others. Assist in scheduling staff and ensure accurate data input into scheduling systems. Maintain communication with patients and families regarding service-related issues. Coordinate with referral sources and establish positive professional relationships. Ensure compliance with state, federal, and agency intake regulations. Document changes in patient status and update service requirements. Process ICD codes, authorizations, and communicate POC changes to the scheduler. Support marketing efforts, including research, email campaigns, and meeting coordination. Provide weekly intake reports to management (referrals, discharges, hospitalizations, staffing changes, etc.). Assist in the hiring and onboarding process for new caregivers. Perform additional administrative and support duties as assigned.
Skill Requirements:
Enjoy solving scheduling challenges and working in diverse environments. Strong written and verbal communication skills. Excellent analytical, organizational, and time management abilities. Attention to detail with a zero-tolerance for errors. Ability to work independently and collaboratively in a fast-paced environment. Ethical and dependable, with excellent interpersonal skills.
Performance Standards:
Maintain 98% accuracy in referral processing and data entry. Ensure all required admission paperwork is completed timely and correctly. Handle all queries and issues within 4-24 hours. Ensure accurate management of insurance authorizations to avoid payment denials. Meet deadlines, demonstrate initiative, and adapt to changing priorities. Uphold professional standards in communication, punctuality, and reliability. Adhere to all company policies and quality assurance procedures.
