Summary Description:
The role of the scheduling coordinator is to ensure that service requests for home care are met in the shortest amount of time possible with consistency in scheduling and supporting team members with their shift preferences and availability. Conducts timely and effective scheduling of assignments to support wellcoordinated service plans, maintain productivity standards and policies by utilizing effective scheduling. Identifies and communicates to Home Care Director barriers to productivity and staffing. Creates, revises and organizes home care team schedules according to the service plan requirements and staff accessibility. Assists Director in the review and/or screening of potential applicants. Responds to resident requests for change in services that may impact schedules. Collects and processes utilization sheets and prepare report to submit to Finance for billing. Manages tasks and duties that support the overall operations of the home care department. Serves as home care assistant or companion in the event of a call out or new unfilled resident request. Uses technology to improve daily operations and service enhancement
Essential Functions:
One to ten specific job related responsibilities that will be used to measure performance. Performs job functions in a manner consistent with the mission and goals of the Loomis Communities. Adheres to The Loomis Communities Compliance and Ethics Program. Demonstrates an awareness of, a respect for and attention to the diversity of the people with whom they interact (persons served, personnel, families/caregivers, and other stakeholders) that is reflected in attitudes, behavior and services. Routinely engages with caregivers to determine availability of scheduling. Completes schedules, staffing assignments, billing and payroll in a timely manner and responds to all home care inquires timely. Prioritize and schedule open shifts; find coverage for all time off requests and any last-minute cancellation. Communicates scheduling changes to residents and staff. Utilizes an automated scheduling system to maintain a calendar of services. Hires, trains, directs and supervises department personnel, including interviewing, overseeing the orientation of new employees, conducting performance reviews, setting objectives and priorities and implementing corrective action in a timely manner. Routinely engages with residents to exceed their expectation with consistency of scheduling and satisfaction with caregivers. Communicates resident issues and needs, reviews daily communication records to ensure prompt resolution of problems to director and others as appropriate Meets all competency requirements, including SAMMS and educational, training requirements.
