An Excellent Opportunity Mountain Valley Hospice is seeking a detail-oriented and responsive individual to manage home healthcare visit scheduling. This role is essential to ensuring timely, efficient care for our patients and clear communication across our care teams.
Key Responsibilities:
Schedule and assign home health aides based on patient needs, aide availability, and geographic location. Coordinate and maintain daily CNA visit schedules to ensure timely coverage. Manage last-minute changes such as call-offs or emergencies, minimizing service disruptions. Respond to incoming calls and messages regarding new needs, schedule changes, and urgent issues throughout the day. Ensure all CNA visits are supported by physician orders and comply with care plans, agency policies, and regulatory requirements. Track visit completion and assist with administrative tasks such as timesheet verification and reporting. Keep clinical and administrative staff informed of schedule updates and relevant changes. Provide front desk coverage at other office locations when the regular staff member is unavailable. Qualifications Previous experience in scheduling, preferably in a healthcare or hospice setting. Strong communication skills and the ability to multitask in a fast-paced environment. Familiarity with electronic health record (EHR) systems is preferred. Excellent attention to detail and organizational skills.
