HOME CARE CLINICAL COORDINATOR/SCHEDULER C SAGE
Eldercare 290 Broad Street, Summit, NJ 07901 Overview We are seeking an energetic, proactive, and organized Care Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing patient care by coordinating services, scheduling visits, ensuring compliance with healthcare regulations, and facilitating communication among patients, families, and staff. This position requires a strong understanding of medical administrative support and health information management to effectively assist in the delivery of high-quality patient care. Responsibilities Coordinate medical scheduling of home care services based on patient needs and staff availability. Adjust patient schedules as needed to accomodate changes in patient conditions Assist with taking inquiry calls from potential clients for home care services. Manage health information management tasks, including filing and organizing patient documents. Collaborate with healthcare providers to ensure seamless communication regarding patient care. Serve as primary point of contact for patients, family members, and caregiving staff. Work collaboratively with nursing staff and support personnel. Requirements Proven experience working in home healthcare environment. Proficiency with medical scheduling software Proactive and problem-solving work ethic. Knowledge of HIPAA regulations, home care regulatory standards, and commitment to maintaining patient confidentiality. Strong organizational and multitasking skills. Excellent communication skills.
