Home Care Agency StaffClient SchedulerKeyResponsibilities:
Schedule caregiver shifts based on client needs and caregiveravailability.o Confirm all shifts with caregivers and clients daily.o Adjust schedules promptly for callouts, absences, or last-minute changes.o Maintain updated and accurate calendars for all active clients.o Record changes in care times, service types, or caregiverassignments.
Communicate with caregivers regarding upcoming schedules,changes, or confirmations.o Notify staff and management of any scheduling conflicts.o Monitor caregiver clock-in/clock-out compliance throughscheduling software.
Report repeated tardiness, no-shows, or attendance concerns tosupervisor.o Update and maintain individualized care plans as clients’ needschange.o Ensure care plans are accessible and understood by caregivers.o Match caregivers to clients based on skill set, preferences, andcompatibility.o Maintain notes on successful caregiver-client pairings andoutcomes.
Conduct periodic check-ins and follow-ups with clients or familymembers.o Document client satisfaction and report concerns to managementArrange for substitute caregivers when primary staff areunavailable.o Ensure no care gaps occur, and all shifts are covered.
