is currently seeking an experienced:
TRANSPORTATION COORDINATOR
– (CNA/EMT) Full Time, Days
JOB SUMMARY
After notification by facility staff of needed appointment, will make all appointments for residents.
Will maintain a calendar of all appointments so that none will conflict.
Will follow all safety guidelines to ensure resident transports are performed in the safest possible manner.
Will maintain required logs of each transport and for each vehicle.
Completes Quality Assurance monitors according to policies and procedures. . Will establish and maintain a maintenance schedule for each vehicle that is used in transports.
Assists in maintaining a good working relationship with themunity, hospital, physician offices and other health care facilities and agencies and assuring coordination of services for our residents.
Perform Receptionist position when necessary
Perform resident care when assigned and appropriate based on certification.
Perform other duties as assigned.
JOB REQUIREMENTS
Must be at least 21 years of age or older, must be of good moral character, and have a genuine concern for the elderly and disabled.
Must have CPR, First Aid Training, and wheelchair safety training. Preferably certified nursing assistant.
Must have skill checklistpleted by Liberty trainer prior to transporting residents. Must have a valid state Driver’s License to operate a motor vehicle with an appropriate driving record.
Must be willing to work with various department directors, patients, visitors, family members, andmunity health care providers.
Must have the ability to plan,anize, and follow-up on work assignments.
Must have the ability to make independent decisions and work well under pressure.
Must have the ability to be flexible in performing different tasks.
