ESSENTIAL DUTIES AND RESPONSIBILITIES

Responsible for receiving cases from intake via contracts or private pay relationships and assign homecare workers to clients assuring that all assignments are carried out. Monitor the Homecare workers compliance with DOH standards in conjunction with the HR Department. Supervise, orient and oversee that quality care is given to our home care population. Participate in the agency’s quality assurance initiative. Supervise and coordinate schedules/shifts for homecare workers; schedule replacement/coverage as necessary; maintain schedule changes; verify all schedules are correct and accurate for all assigned cases. Initiate the placement of home care services within the prescribed measurable timeframe. Document all communication regarding implementation of service for all initial cases, change in authorized hours or terminations of services. Coordinate Plan of Care (POC) Orientation for homecare workers with the nursing team and follow up with employee to ensure a copy of POC for case(s) assigned was provided. Ensure client service are scheduled as prescribed on authorization documentation. Review admission and discharge of clients to ensure agency does not bill inappropriately Monitors HCWs task completed for all CHHA clients and ensure that all task matches clients POC. Generates and review exception reports, i.e. no call in/out, no show, etc. daily and ensure that all concerns/issues are addressed in a timely fashion. Follow up on client/homecare workers complaints. Review open scheduling orders in the available system report. Document incident reports for clients and homecare workers. Monitor homecare worker attendance to ensure coverage. Conduct reminder calls to ensure employees attend scheduled in-service and physical appointments. Report and accurately document notes in organization’s tracking system. Provide support, assistance and encouragement to HHA’s in the use of the Sandata Mobile Connect and Care Connect applications. Serve as On-Call Coordinator and back-up for other Care Coordinators as needed. Responsible for conducting annual homecare worker evaluations. Report any homecare workers compliance issues as related to evaluation, in-service, annual physical requirements to Care Coordination Supervisor. Maintain effective working relationships with co-workers and supervisory staff across all departments. Attend case conferences as needed. Attend agency meetings and staff development training. Other related duties as assigned.

COMPETENCIES 2-5

years of experience in the homecare or related field Bilingual (English and Spanish) preferred Problem solving abilities while multi-tasking; ability to exercise sound judgment Excellent verbal and written communication skills Excellent customer service and organizational skills Knowledge of home care concepts, practices and procedures within a LHCSA Ability to maintain confidentiality at all times Commitment to organization’s mission, vision and values Moderate to strong computer proficiency using standard office software programs required, in particular Microsoft Office; ability to utilize technology in daily work; experience with web-based applications
