POSITION OVERVIEW
Palmetto Family Homecare is currently seeking an experienced Care Services Coordinator to join our Columbia office team. The Care Services Coordinator provides administrative and logistical support to ensure high-quality home care delivery. This role is responsible for scheduling, intake coordination, caregiver support, and maintaining open communication with clients and field staff. The Care Services Coordinator plays a critical role in maintaining compliance and continuity of care.
JOB RESPONSIBILITIES SUMMARY
Client Coordination & Support Respond to all incoming client referrals and service inquiries Maintain and meet established client leads, calls, etc. Schedule and support intake assessments in collaboration with the nurse Maintain client records & service authorizations in company HSRM system Communicate information regarding the patient to the Nurse Supervisor and/Care Services Manager; making agency administrative staff aware of additional patient needs as necessary. Participate in staff meetings, public education and provider training sessions, as appropriate. Develop and maintain ongoing communication with case managers, discharge planners, and physician office staff, conducting frequent presentations, both formally and informally, to build a strong network of referrals. Scheduling & Staffing Coordinate caregiver/client schedules to ensure full coverage of service hours Review and respond to reports via HRSM portals Communication & Documentation Maintain thorough communication with clients and caregivers Assist with orientation as needed and contribute to employee retention efforts Document all supporting records in accordance with regulations of applicable regulatory agencies. Operational Support Answer office and on-call phones as assigned Monitor system portals for new referrals and updates Assist with uploading and maintaining agency documents
Skills & Competency:
Exceptional customer service and communication abilities Strong organizational and time management skills Proficiency in care management software (e.g., AxisCare, Phoenix) Ability to multitask and prioritize in a fast-paced environment Problem-solving and conflict resolution skills Commitment to confidentiality and compliance (HIPAA)
JOB REQUIREMENTS
Minimum of 1-3 years in a healthcare, home care, or administrative support role Associate’s degree or certification in healthcare administration or related field preferred Experience working with scheduling or EHR software preferred Exceptional verbal and written communication skills Ability to build rapport and develop working relationships with referral sources Strong presentation and marketing skills High school diploma or equivalent required Must have a valid state driver’s license and appropriate auto insurance.
