Client Care Coordinator 3.9 3.9 out of 5 stars Ridgefield, CT 06877 We are looking for a dedicated Client Care Coordinator to join our non-profit organization in Ridgefield, Connecticut. In this role, you will play a vital part in fostering positive relationships with clients, ensuring their needs are met, and coordinating care services effectively. This position offers an opportunity to make a meaningful impact by delivering exceptional support to clients and their families.
Responsibilities:
Conduct regular follow-ups with clients to address concerns and confirm satisfaction with services. Maintain and update comprehensive client files with accurate medical and personal information, adhering to confidentiality standards. Serve as the primary liaison between clients and caregivers, ensuring the most suitable caregiver is matched to meet client needs. Coordinate schedules and facilitate clear communication between clients and caregivers for seamless service delivery. Collaborate with team members to enhance processes and improve the overall client experience. Provide empathetic support and assistance to clients and their families, addressing any challenges or special needs. Ensure compliance with organizational policies and relevant regulations while managing client care. Monitor caregiver performance and client feedback to ensure high-quality service delivery. Proven experience in customer service or client care coordination roles. Strong administrative skills with attention to detail and accuracy. Excellent communication skills to effectively interact with clients, caregivers, and team members. Ability to manage schedules and coordinate multiple tasks simultaneously. Demonstrated empathy and understanding when working with clients and their families. Proficiency in maintaining confidential records and adhering to compliance standards. Familiarity with non-profit operations or a passion for working in mission-driven environments is a plus. Previous experience in patient care coordination or similar roles is highly desirable.
