Position Summary:

We are seeking a compassionate, organized, and tech-savvy Medical Assistant to join our call center team, specializing in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) . This position involves working directly with patients over the phone and via digital platforms to help manage chronic conditions, track health metrics, and coordinate care services.

Key Responsibilities:

Conduct monthly and as-needed outreach calls to patients enrolled in CCM and RPM programs. Collect, document, and monitor patient-reported health data (e.g., blood pressure, glucose levels, weight) via connected devices and platforms. Identify abnormal readings or symptoms and escalate to appropriate clinical staff. Assist in developing and updating patient care plans under clinical supervision. Educate patients on their chronic conditions, treatment plans, and proper device usage. Coordinate appointments, medication refills, referrals, and other patient needs. Maintain accurate, timely documentation in electronic health records (EHR). Work collaboratively with nurses, providers, and other care team members to ensure quality, continuity, and effectiveness of care. Adhere to HIPAA and company policies regarding confidentiality and patient safety.

Qualifications:

Required:

Certified Medical Assistant (CMA, RMA, or equivalent). High school diploma or GED. Minimum 3 year of experience in a clinical or call center setting. Strong interpersonal and communication skills. Proficiency with EHR systems and Microsoft Office. Comfortable working with RPM tools and technology.

Preferred:

Experience in chronic disease management (e.g., diabetes, hypertension, CHF, COPD). Not Required, but a plus: Bilingual (Spanish, Mandarin, etc.). Familiarity with platforms like CareSimple, HealthSnap, SmartMeter, ChronicIQ, ThoroughCare or similar. Experience with programs such as, Chronic Care Management, Remote Patient Monitoring, Transitional Care Management, etc
