Job Description
Facilitator – Assisting Healthcare Providers Phlebotomy Jobs Lexington, NC 27295 The Facilitator assists healthcare providers with providing healthcare options and pre-planned care initiatives to help patients make informed decisions about their personal healthcare goals. The Facilitator will work in various healthcare facilities, such as nursing homes and acute rehab facilities. The Facilitator role is an exciting way to assist patients with making informed decisions about their healthcare options and choices by coordinating telehealth sessions between healthcare providers and patients.
Responsibilities:
The Facilitator will be expected to work alongside an assigned healthcare provider, visiting a set number of patients each shift, to help facilitate dialog and communication between providers and patients. The Facilitator will also be responsible for completing the desk audit task provided by FloatR at the end of each shift, accounting for the number of patients seen and the name of the provider they’ve worked with.

some travel time may be required and may be compensated for upon admin approval.
Required Qualifications:
Highschool diploma or equivalent is required Reliable means of transportation to facilitate travel Ability to utilize a smartphone or tablet, including utilizing the FloatR App and uploading documents and files. Must own a smartphone or tablet Strong communication skills and the ability to interact professionally with patients and healthcare team members Ability to stand on one’s feet and walk throughout each healthcare facility. Must be courteous, punctual, and professional. Must have the appropriate medical clothing such as scrubs. Not Required, but preferred: Has at least 2 years of experience in the healthcare industry and patient care. Willingness to travel. Travel time may be compensated depending on distance and administrator approval. The willingness to stay overnight in a hotel or travel long distances is a plus. (lodging would be provided) Experience with communicating healthcare options and terminology to patients.
