Perform an intake and needs assessment of a client’s immediate needs and goals in conjunction with building a relationship for ongoing help and support. Develop an individualized service plan and assist the client in taking the initial steps toward progress in defined areas (job hunting, housing, education, etc.) Develop successful partnerships with vital social service organizations and businesses to provide clients with needed referrals (i,e., DSS, Health Department, adoption agencies, homeless shelters, domestic violence shelters, etc.) Support a client in navigating the application process to referring agencies. Monitor the client’s progress through frequent follow-up, possibly for as long as 2 years. Keep detailed records of client-related expenses and required documentation required for reports. Client Services Coordinator Responsibilities Work with Health and Client Services departments to serve with compassion and excellence. Check-in clients, schedule appointments, make appointment openings, etc. Open and close client area, check voicemails, answer the phone, transfer calls, and greet visitors at the door. Prepare appointments, send text reminders, follow up with carry clients, and contact them as needed. Keep track of various paperwork and make copies when needed; remain up-to-date on data entry Help maintain the cleanliness of client areas Serve if needed as an advocate during client appointments. Articulate and utilize TPN’s resources and offer community resources, as needed. Work with the Volunteer Coordinator to recruit and develop volunteers and volunteer teams for administration, classes and mentorship, and peer advocacy; continually find new ways to train and empower volunteer team members and leaders to lead other volunteers for effective ministry. Assist Spanish Client Services Manager with classes and mentorship programs for Spanish-speaking clients.
