We’re currently seeking an experienced and motivated Client Care Coordinator to join our team in Livingston, NJ. This is a key position that ensures caregivers are scheduled efficiently, clients’ needs are met, and all documentation is accurately maintained.
Position Overview:
The Client Care Coordinator is responsible for coordinating caregiver schedules, communicating with clients and families, documenting all interactions, and supporting the Human Resources and Nursing teams to maintain compliance. This full-time role is ideal for someone who thrives in a fast-paced, team-oriented environment and is dedicated to providing exceptional client service.
Responsibilities:
Assign and maintain client schedules with caregivers Communicate schedule changes, call-outs, and appointments with clients and families Document client and caregiver communication in the scheduling system Match caregivers to clients based on skill set, experience, and availability Coordinate staff coverage for unscheduled absences or time off Track hospital admissions and discharges; update client information Support HR and Nursing with evaluations, supervisory visits, and compliance tracking Maintain and participate in the on-call schedule with other team members Perform general office duties: answering phones, filing, ordering supplies, etc. Assist with marketing efforts and distributing recruitment flyers to designated sites Support Certified Home Health Aide interviews and orientation sessions
Qualifications:
Prior experience in a home care or healthcare coordination role Strong communication skills and fluency in English Ability to multitask and stay organized under pressure Professional demeanor and reliability Comfortable working with scheduling systems and data entry Must be willing to take part in after-hours and weekend on-call coverage
