Job Description

Client Care Coordinator 3.6 3.6 out of 5 stars Cookeville, TN 38505 Training will take place in Brentwood, TN for 2-3 weeks Under the direction of the General Manager, provides continuity of care for clients. Facilitates the provision of uninterrupted in-home care and support services to clients. Assists in the organization of family resources for the effective care of the client. Routinely interacts well with clients, family members, community providers / referral sources, care managers, and other third-party payers. Is responsible for conducting the initial company presentation to the family/client working towards converting the prospect into a client. Some soft sales required.

Qualifications:

Experience in coordinating health care needs. Higher education is a plus. AS/BS degree in social work, LPN or similar field desired. Ideal candidate would be an LPN or Social Worker Must possess and demonstrate excellent communication skills as well as positive professional, business image. Ability to multi task is a must.

Essential Functions:

Assesses prospective clients for service needs. Convert the prospect into a client and establishes initial client records. Quality Assurance Visit to monitor the client’s condition and environment on scheduled intervals. Responsible for determining type and duration of services necessary to meet client needs. Assist in matching caregiver qualifications and availability to client’s needs. Conduct supervisory field visits on caregivers and provide documentation to the HR Mgr. Follow-up with clients and caregivers to ensure satisfaction of services. Serves as a liaison between the client, caregiver, scheduling and human resources mgr. Engages community referral sources and marketing. Local Travel required Participates in client conferences as needed. The employee will be expected to perform other reasonable related duties as assigned.

Knowledge, Skills, Abilities:

Must possess excellent verbal and written communication skills as well as time management, and organizational skills. Positive professional, business image. Proficient computer skills in an Apple environment (Word, Excel, Power Point) Client-centered service experience and ability to work with diverse populations Detail-oriented Self-manager/self-motivated Excellent interpersonal skills and ability to persuade others Ability to work flexible hours required, some evenings and weekends Ability to be flexible and to prioritize in complex situations. Decision-making skills and ability to handle pressure

Schedule:

8 hour shift Monday to Friday On call Application Question(s): Training will take place in Brentwood, TN for 2-3 weeks, will you be able to commute for this time period? Interviews will be two parts, Virtual and then in person in Brentwood, TN, will you be able to participate in both?
