Job Description

Case Management Writer 2.8 2.8 out of 5 stars 3543 Habersham At Northlake, Tucker, GA 30084 Professional Case-Management Services of America (PCSA) currently has a full-time Case Management Writer in our Tucker, GA office. General Duties Documents ISP meetings via support notes to ensure that the information is captured Updates individual demographics information in IDD as needed Updates manager on a weekly basis regarding what ISP’s are still pending. Updates Manager regarding any needed Region to Region transfers, RCR, and version changes that are needed to individuals that is being covered for ISP purpose. Ensure that all ISP forms are uploaded to docs in a timely manner. Ensures that the SIS is within the timeframe and updated every 3-5 years as per policy. Ensures that the HRST is updated by the provider agency and Region as required according to 90/120 day policy. Completes other duties as assigned. ISP Responsibility 100% of all ISPs will be scheduled and ISP meeting will be held at least 70-75 days prior to consumer birth date or current ISP expiration date. Complete monthly ISP schedule or follows the calendar in place for meetings that have been scheduled, and reviews with manager weekly. Completes ISP in IDD system within 4 business days of ISP meeting. Completes ISP in IDD with no more than two returns for corrections. Make corrections to ISP within 48 hours of entry in IDD system. Tracks all ISP until they have been approved by regional office staff 100% of the time. Ensures that 100% of ISP goals utilize person centered (SMART) tools and document evidence of use in the ISP. Ensures that each participant’s ISP is aligned with the birthday rotation. Ensures that all LOC reviews/recommendations in the ISP are addressed in a timely fashion.
