Team Lead – Medically Enhanced Supportive Housing (MESH) New Heritage Recruiters Burlington, NJ 08016 Job Overview Under the supervision of a director, the Team Leader is responsible for the management and administration of assigned Community Support Services components. Program Description Medically Enhanced Supportive Housing(MESH) is an advocacy & housing service for adults with a serious mental illness and chronic/serious medical diagnoses. Duties and Responsibilities Directly responsible for the monitoring for efficient & effective services as assigned by the Program Director. Recruit, hire and supervise the staff, Clinical supervision must be in conjunction with the Program Director. Responsible for ensuring MESH staff understand the goals of the program and that all are working toward consumer recovery and independence. Oversee the implementation of Individual Rehabilitation Plans by direct care staff with input from the treatment team members. Their signatures on the Plans indicate agreement of treatment team. Coordinate with other community services that work with the MESH consumers, such as community hospitals, entitlement agencies, community medical providers, and other mental health providers. Consult and coordinate with Program Director & direct services staff re: admissions, transfers & discharges from State Psychiatric Hospitals. Responsible for ensuring all regulations are followed in these processes. Oversee the expenditures of MESH, keeping the Director informed of spending and needs. Oversee the management of housing leases for consumers to ensure consumers and their staff assist in maintain community housing. Emergency on- call availability to MESH staff, 24/7. Position Requirements Master’s Degree in human services field LSW (In process of getting fully licensed), LAC, LCSW, or LPC Two years in a supervisory position preferred Five years’ experience in mental health Ability to work flexible hours, provide on-call response Valid driver’s license required Schedule Monday – Friday, 8:30am – 4:30pm (+ call responsibilities)
