Admissions Manager St. Margaret’s Center 27 Hackett Boulevard, Albany, NY 12208 Admissions Manager for Pediatric Skilled Nursing Monday – Friday Days $33.85 – 50.78/hour depending on experience The Admissions Manager l eads the networking and admissions process with the goal of maintaining 100% occupancy with accompanying waitlist. Locates and gathers information on potential residents for appropriate St. Margaret’s Center placements. Works with hospital discharge planners, case managers, residents/families, and other referral sources to expedite placement and admission to the facility. St. Margaret’s Center , an affiliate of Center for Disability Services, is a pediatric and young adult 94-bed skilled nursing facility, located in Albany, NY. Effectively markets services delivered by St. Margaret’s Center to hospitals, prospective residents, families, social service localities, insurance companies, service coordination providers and, the general public Networks with
Hospitals/Community:
serves as the designated staff member for strengthening communication and marketing services to the community Leadership role in working with Foundation division to develop effective marketing plan and associated marketing materials Maintains routine contact via phone, email, mailings, and in-person with referral sources throughout NY and neighboring states (includes potential for air travel) Maintains competitive advantage through regular assessments and strategize competitor facilities’ marketing activities and census trends In consultation with Director of Nursing and RT Manager, when appropriate, arranges clinical assessment of all applicants for admission, and makes recommendation to Nursing Home Administrator, Medical Director and providers on appropriate fit for permanent and respite placement Secures an accurate and complete PRI (Patient Review Instrument prior to admission Maintains documentation of PASRR (PreAdmission Screening and Resident Review) according to state and federal regulations, including Level 2 reviews with OPWDD districts Works with Director of Business Services to evaluate cost of service for prospective resident care Acquires comprehensive clinical and social documentation to support accurate Committee review for potential admission Manages the admission process for new residents into the facility including completing necessary documentation. Collaborates with Social Worker and other IDT members to ensure that the comprehensive intake process is completed Communicates with physicians regarding clinical status and care plan of prospective new admissions Verifies payor sources and collaborates with Director of Business Services that payer sources are current and valid Participates in 3rd party payor rate negotiations as necessary to establish maximized appropriate rate per resident care needs Ensures necessary documentation is obtained, maintained, and submitted as required to 3rd party payors for continuous revenue flow Monitors and maintains referral information including statistical data for analytical purposes; reports to the Nursing Home Administrator at least monthly Maintains referral data in electronic health record Reviews and amends facility P&Ps related to Admissions as defined by state and federal regulations Leads and facilitates Admissions Committee in compliance with facility P&Ps, as well as, state and federal regulations Assists the Discharge Committee as needed and as back-up in the absence of social worker services Develops/maintains applicant wait list with all necessary/current information, reporting to Admissions-Discharge Committee Tracks and maintains Respite utilization records to ensure compliance with state and federal requirements Timely and proactive communication to department heads and appropriate staff of projected changes, i.e., admissions, bed changes, and industry trends or changes, as soon as possible Tours prospective residents, their families, and referral sources through facility Leads Admissions process including meeting with new residents and their families to facilitate a smooth transition including, accurate and comprehensive completion of Admissions agreements, consents, insurance and other required documentation Facilitates Pre and Post-Admission Team meetings to evaluate the admission process Facilitates transition and interface for residents and families with Social Services and Nursing Management staff Fulfills Administrative On-Call responsibilities per rotation with other colleagues Manages clinical updates and documentation, as required, to health insurance companies for reauthorization and continuations of stay in the nursing facility Manages referrals from all sources (i.e. phone, email, and electronic referral systems) Acts as a point of contact with hospital case managers for re-admission of residents from acute settings Benefits for St.
