CARE COORDINATION SPECIALIST

We are looking for an out-going individual to join our team as a Care Coordination Specialist. This position requires an individual who is a communicator and people person with the ability to meet deadlines and work independently, while meeting team goals. Responsibilities include case management and supervising and scheduling direct care staff. Applicant must have excellent written and verbal skills and the ability to manage time well. An electronic device with access to the internet, knowledge of Word and Excel, reliable transportation and the ability to be on-call are required.

A B.A/B.S.
