Adult System of Care Specialist 5.0 5.0 out of 5 stars Miami, FL 33126 • Hybrid work Thriving Mind South Florida is seeking an Adult System of Care Specialist. This position provides mental, substance and behavioral health services to adults. The system aims to help adults achieve their optimal level of self-sufficiency and independence. The position will report to the Adult System of Care Manager.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following, as well as other work-related duties as assigned: Serve as the liaison to the South Florida State Hospital and other state mental health treatment facilities Coordinate monthly discharge tracking meetings with South Florida State Hospital and community providers Work with community providers to assist with the discharge planning process for individuals exiting state mental health treatment facilities Review clinical assessments to determine appropriate treatment referral. Manage waitlists and provide prior authorizations for adult substance abuse and mental health residential treatment including short-term residential treatment (SRT). Review and make recommendations for placement into the Florida Assertive Community Treatment Teams (FACT). Manage referrals for FACT services and provide oversight and technical assistance to FACT Team providers Coordinate Forensic placement and services. Collect data, design reports, and interpret information for monthly, quarterly, and annual reports as necessary. Review, track, and manage client files. Track and monitor client progress in treatment. Review monthly continued stay requests to determine if clients are eligible for an extension on length of treatment as well as monitor the appropriate paperwork being sent in with such requests. Monitor agencies’ contractual agreements to ensure compliance with contract mandated program goals and requirements. Research and make recommendations concerning funding requirements for current and project programs. Represent the Network at workshops, on task forces and at various planning meetings when requested. Perform other related duties and special projects, including community-based and/or agency events as assigned. Assist providers with balancing access, quality of care, and cost efficiency. Provide/Coordinate and make recommendations for training and technical assistance for provider agencies. Advise provider agencies on Evidence Based Practices (EBPs); and assist providers with implementation of those EBPs. Assume other tasks as assigned by Provider Services Manager.
QUALIFICATION REQUIREMENT
Knowledge of the adult system of care. Knowledge of admission criteria and clinical assessment instruments.
EDUCATION AND/OR EXPERIENCE
Master’s Degree & Licensure preferred; bachelor’s degree with 2-year experience in related field.
LANGUAGE SKILLS
: Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints.
FINANCIAL SKILLS
Ability to administer multiple contracts and work effectively with auditors.
REASONING ABILITY
Ability to travel to various meetings for the agency. Excellent interpersonal skills, communicate effectively as a team player with peers and management.
