, 8 HR Day Shift, POSITION SUMMARY The Life Care Specialist role works as a part of the patient care team to empower patients through the provision of medication (opioid) education, mental wellness skills, and alternative pain management strategies. Partners with nursing staff, case management staff, etc. to ensure continuity of care for specific patient population. This position is supported by a partnership between SGMC Health and CWC Alliance.
EDUCATION
High School graduate or equivalent required Associates or Bachelor’s degree in healthcare related field preferred Professional licensure or certification in healthcare related field preferred
EXPERIENCE
Experience working with Electronic Medical Records (EMR) strongly preferred Previous work (paid or volunteer) in advocacy roles &/or community outreach Experience with providing training and education in a professional setting Trained in the Community Resiliency Model (CRM)Ⓡ
KNOWLEDGE, SKILLS, AND ABILITIES
Compassionate team player with a positive attitude and empathetic outlook Healthcare experience in a patient-serving setting (e.g. hospital or clinic) Excellent organizational and communication skills Working knowledge of Microsoft Excel
WORKING CONDITIONS – ADA INFORMATION
Works primarily in moderate to fast-paces hospital setting. Requires extensive walking during shift. Physical demands include moderate lifting of 25+ lbs., sitting, standing, stooping, bending, pushing objects (30+ lbs.). Likely to be exposed to blood and body fluids and infectious diseases on a daily basis. Uses general office equipment in performance of duties, including but not limited to computers, printers, fax, telephone & nurse call systems. Interacts daily with persons under stress, having complex complaints, and/or with whom there is a need to respond to questions, problems, or gain cooperation.
