DESCRIPTION

1. Responsible for assisting with and implementing special education instructional programs, specifically dealing with transitions.
2. Responsible for providing direct service to students with disabilities aged 18

3. Demonstrate in-depth knowledge of transition programming and processes for students with disabilities at the secondary level.
4. Work with transition specialist to coordinate and cultivate relationships between school and community work based learning opportunities.
5. Participate in reevaluation and annual review meetings for students on caseload and write Individualized Education Programs (IEPs), Transition Planning Forms, Age of Majority Forms, and Summary of Performance forms.
6. Assist in the guardianship process for students and families as needed.
7. Complete and coordinate data-informed progress reports from service providers for all students, according to the schedule set by host school, Collaborative, and special education regulations.
8. Develop and implement schedules for students’ services in accordance with their IEPs’ service delivery; coordinate schedules with specialists; work with general education teachers to schedule students’ general education classes and activities.
9. Select appropriate instructional programs and methodologies, plan, and implement instruction that matches students’ IEP goals and objectives.
10.Develop and implement curriculum aligned with Frameworks and Common Core Standards.
11. Provide differential instruction and interventions in the classroom.
12. Implement appropriate behavior management techniques and/or behavior support plans.
13. Maintain and use current technology and adaptive equipment and materials.
14. Develop and implement classroom and curricular accommodations and modifications.
15. Assess and maintain data on progress of all students on caseload and differentiate student goals and instruction accordingly.
16. Determine student educational needs based on appropriate assessments, write reports and present at annual and reevaluation Team reevaluation meetings.
17. Develop teaching assistants’ schedules in accordance with students’ IEP service delivery grids and needs.
18. Consult with special education teaching assistants and tutors; including planning, coaching, modeling.
19. Provide supervision of assigned special education teaching assistants.
20. Provide evaluative feedback and input regarding teaching assistants, including completion of biannual evaluation forms, to Program Administrator.
21. Collaborate with students’ special education Team to address transition-related issues and skills and to implement transition services according to the Transition Planning Form (TPF).
22. When appropriate, integrate MA Career/Vocational Technical Education Frameworks into content across the curriculum.
23. Consult with parents/guardians, general education teachers, therapists, school nurses, consultants, and teaching assistants regarding planning and students’ program needs.
24. Conduct parent/guardian-teacher conferences as and when needed.
25. Provide communication to parents/guardians of students on caseload (e.g., parent-teacher conferences, email, telephone contact).
26. Participate in Open House for parents/guardians, staff meetings, and professional development opportunities, as appropriate.
27. Complete budget requests for the Collaborative budget; order necessary materials for instruction and assessment.
28. Given training by a Registered Nurse and/or training videos, the teacher will use an EpiPen and/or Vagal Nerve Stimulator with the student for whom they were prescribed.
29. Attend Collaborative-wide and specific program staff meetings, as scheduled by the Program Administrator and/or Executive Director.
30. Keep abreast of the latest trends, materials, research, and best practices related to the field.
31. Participate in professional development opportunities to increase knowledge base and skills, as well as maintain licensure.
32. Maintain the high level of confidentiality and professionalism expected of all faculty.
33. Perform other related tasks or duties at the discretion of the Program Administrator and/or Executive Director.
