ROLE ANDRESPONSIBILITIES

? Assessing the medical needs of the patient tounderstand their full clinical picture ? Review health insurancebenefit plans to determine if coverage options meet the needs ofthe patient. ? Coordinate patient care options with Hospitalpartner and providers post discharge including but not limited topediatric hospitals, home health, pharmacy, durable medicaltreatment. ? Facilitating aspects of the treatment plan, such ascare coordination and discussions with the patient and family ofthe patient. ? Recording all case information includinginteractions with internal and external teams. ? Building strongrelationships with the medical team throughout the case managementprocess. ? Communicating with other healthcare providers,specialists, and community services to ensure coordinated care andseamless transitions. ? Responding to clinically complex situationsthat may arise with patients within 1-2 business days. ?Representing the patient’s needs and advocating for access tonecessary services and resources. ? Attend as needed meetings withhealthcare team members to discuss patient medical needs andobstacles in receiving care. ? Consistently demonstrate the abilityto successfully operate in problem-solving mode whenever challengesor concerns arise. Job Requirements Bachelors in Nursing or SocialWork with clean and active MA license. 3 years of case management,managed care, care coordination type experience.
