Inspiring Success, Creating Community Memory Care Resource Specialist Job Summary:

The Memory Care Resource Specialist will provide professional support and guidance to residents and families of Northbridge’s three Memory Care assisted living communities located in Brunswick, Westbrook, and Wells Maine. The Memory Care Resource Specialist will travel to assigned communities providing support to our residents and families.

Responsibilities:

Assist with Sales process providing emotional support and guidance to residents and families.
Assist with outreach as directed by Sales Director and/or Executive Director.
Provide one on one family support as needed using cognitive behavioral, solution focused and mindful approaches.
Takes a participatory role in resident assessments, development of care plan and care.
Follow-up with new Resident and family post move -in 30/60/90 days.
Co-facilitate monthly family support groups
Assist with Hospice conversations and lead bereavement support following residents’ passing.
Ability to provide behavioral interventions, crisis counseling.

Qualifications:

Bachelor’s degree in social work, Gerontology or related field (preferred).
Licensed to practice counseling and/or nursing in Maine (LICSW, MSW, RN/LPN) preferred
Must have a minimum of 2-4 years’ experience in Memory Care working with elders or disabled individuals as well as Assisted Living experience.
Strong understanding of the senior living market and the needs and concerns of seniors and their families.
Must be able to travel, generally a 60-mile radius and work flexible hours, including evenings and weekends as needed.
