Responsibilities Share personal story to assist/support the member; Perform general outreach, engagement, and follow-up with members; Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation); Sharing information with the care manager and other members of the care team on the member’s circumstances; Support the care manager in assessing and addressing unmet health-related resource needs; Support member with mental and physical health needs; Other duties as assigned.

Knowledge, Skills and Abilities:

Strong interpersonal and written/verbal communication skills Proficient in Microsoft Office products (such as Word, Excel, Outlook, etc.) Good organizational skills to prioritize duties to meet deadlines
