Job Description

POSITION SUMMARY

Cross-trained position including Utilization Management/Care Management roles. Responsible for reviewing documentation in the chart for appropriate status and coordinating the care of patients meeting case management criteria across a continuum of care. Ensures and facilitates the achievement of quality, clinical and cost outcomes. Assesses, plans, and coordinates the discharge planning process for patients, as well as arranging post-acute care services for patients in a timely healthcare delivery. EXPERIENCE
Minimum Required:
ED Care Management Specialist:

Graduate of an accredited school of professional nursing, with current license to practice as a Registered Nurse in North Carolina. Thorough knowledge of nursing care process from admission through discharge, gained through several years of clinical experience as a RN.

Preferred:
ED Care Management Specialist:

Baccalaureate Degree in Nursing and previous experience as a Care Manager or Utilization Review Specialist. Knowledge of utilization review and discharge planning methods and procedures. Understanding of clinical pathways and disease management models.

ADDITIONAL SPECIALIZED KNOWLEDGE/SKILLS

Incumbent must demonstrate an ability to apply age-appropriate principles of communication and clinical practice relevant to neonatal, infant, pediatric, adolescent, adult and geriatric patients. Incumbent must demonstrate competence in the use of equipment and application of procedures before being allowed to perform independently.
