ADRC Resource Center Manager General Overview:
The Aging & Disability Resource Center (ADRC), a program of the NH Department of Health and Human Services, collaborates with the Partnership for Public Health (PPH). As New Hampshire’s official Aging and Disability Resource Center and NHCarePath Full-Service Access Partner, the ADRC serves individuals of all ages, income levels, and abilities. They facilitate connections to long-term services and support, family caregiver resources, options exploration, and information on Medicare and Medicaid. The primary role of the Caregiver Specialist is to provide assistance to family members and others who provide day to day care for another individual without compensation. Services include information, counseling, education and funding for eligible respite care. Those served through the program include family caregivers of frail older adults, younger adults with disabilities, grandparents raising their grandchildren, and caregivers of people diagnosed with Alzheimer’s disease and dementia. The primary role of Long Term Care Medicaid is to empower the patient/caregiver to: 1) become an active participant in their care; 2) assist in the application process for Long Term Care Medicaid. 3) provide support to patients/caregivers with complex needs that may include hospital visits, home visits and follow up phone calls. Responsibilities (include but not limited to) : The Caregiver-LTSS will work collaboratively with a team of staff in all programs including the Caregiver program, Long Term Care, Medicare and the Veteran Directed Program. Assistance may include information, assessment, intake, clerical and outreach. Those served through the program include family caregivers of frail older adults, younger adults with disabilities, grandparents raising their grandchildren, and caregivers of people diagnosed with Alzheimer’s disease and dementia. Works with caregivers to do assessments, create plans of care, develop budgets, and assist in implementing services that may be made available through NHFCSP and other community support services. Works with clients to do assessments and assist in implementing services that may be made available through DHHS/LTSS and other community support services. Works with the fiscal management agency (currently Gateways Community Services) to ensure that payment for services occurs in a timely and appropriate manner. Develop, implement and conduct outreach and public education sessions as needed, at a minimum, per scope of services with an emphasis on providing and distributing ADRC outreach information in the respective geographic areas. Participate as part of the ADRC team to provide information, referrals, assistance, and follow up to clients through home and community visits, telephone, walk-in, group sessions, or email as needed/appropriate. Assures all electronic client records are maintained accurately
