Transitional Care Specialist – Primary Care

The Opportunity Join our team as a Transitional Care Specialist and help patients navigate the critical period after a hospital or ER visit. In this role, you’ll support patients as they transition back to their Advanced Primary Care team, focusing on post-discharge outreach, care coordination, and reducing the risk of readmission. You’ll collaborate closely with providers, hospitals, and community resources to ensure patients get the right care, at the right time. This is a great role for someone who enjoys patient interaction, is highly organized, and wants to make a real impact in care coordination. Your Impact Monitor daily patient admission/discharge alerts and hospital systems for updates Reach out to patients within two business days of discharge (2-3 call attempts) Conduct telephonic assessments to identify patient needs, risks, and barriers to care Review discharge instructions, assist with medication reconciliation, and schedule follow-ups with primary care Coordinate with providers, home health agencies, and community organizations for needed services Ensure care plans, referrals, and outreach activities are documented in the EMR Address care gaps and contribute to improved health outcomes as part of a multi-disciplinary team Who You Are 3+ years of experience in a healthcare or medical office setting 2+ years as an LPN or Medical Assistant experience Familiarity with electronic health records and Microsoft Office Strong organizational skills and ability to manage multiple priorities A problem-solving mindset and desire to improve care for vulnerable populations Flexibility and a proactive attitude in a dynamic environment #INDCOR1 Why ConvenientMD?
