Program Director Travel or Special Conditions/Requirements:

Traveling required The CTI Program is an evidence-based phase-based, time limited program designed to assist patients in accessing needed services during critical transition periods.

QUALIFICATIONS/EXPERIENCE
  • High School Diploma or equivalent
  • Certified Peer Advocate (CPA) or Certified Recovery Peer Advocate (CRPA) preferred, but provisional peers will be considered and must become certified within 1 year of hire.
  • At least 2 years of work experience providing peer support to individuals with serious mental illness, in recovery, homelessness or have experienced trauma.
  • Graduate of a recognized peer specialist training program such as the Howie the Harp Peer Advocacy and Training Center Peer Specialist Program
  • Lived experience with mental health, substance use, homelessness or trauma events.
  • Proficiency with computer and electronic and health records
ESSENTIAL & CORE FUNCTIONS
  • Carry a caseload of 15.
  • Develop person-centered care plans.
  • Share knowledge of community resources
  • Provide linkage to resources
  • Engage individuals in skill building
  • Offer crisis management
  • Collaborate with other providers
  • Assist individuals in navigating complex systems
  • Work with informal supports.
  • To help individuals receive the level of and/or have access to the services/supports they choose.
  • To monitor and track progress of individuals in assigned caseload; participate in the development of individualized rehabilitation plans.
Travel or Special Conditions/Requirements:

Travel Required

  • Assess and provide services to clients to address housing, income support, education and vocational training, social supports, employment, and primary care needs
  • Assist clients with linkages to and systems navigation within the behavioral health, human services, and criminal justice systems as appropriate
  • Act as an advocate and liaison for clients in accessing resources to support service plan goals.
  • Use motivational interviewing, recovery, and trauma-informed approaches when delivering peer services
  • Involve family and significant others in the client’s services with the team
  • Provide individual peer support, wellness counseling, harm reduction counseling based on motivational interviewing
  • Provide culturally competent services in accordance with IDCC policies and practice
