SUMMARY:
The Clinical Specialist – Infusion Liaison is responsible for the development, education, implementation and coordination of infusion home care plans. Ensures that quality infusion patient care is delivered efficiently, and consistently by agency infusion-trained nurses. This activity is largely done in facilities, preparing patients for their admission to home care services upon their return to their places of residence.
RESPONSIBILITIES
- Participates in the development and maintenance of current operational policies and procedures related to Infusion programs.
- Participates as needed in research projects based on data generated from the continuous quality improvement programs and clinical observations.
- Act as liaison with the medical community at large regarding the Rochester Regional Health Home Care infusion programs.
- Performs direct care and documentation for select patients and caregivers that require specialist expertise.
- Maintains current clinical knowledge in areas of infusion nursing. Perform co-visits with home visiting staff to observe competency and assist with complex infusion cases
- Participates on agency committees as assigned.
- Manage and evaluate the care deficit in complex cases.
- Participates and represents agency in community based programs. Practice according to agency standards and follows applicable policies and procedures.

- Regular and reliable attendance is required.
REQUIRED QUAILFICATIONS
- Diploma or Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
- Registered Nurse license in New York State.
PREFERRED QUALIFICATIONS
- Minimum 3 years related experience preferred
- Strong IV skills preferred
- Home Care experience preferred
- Demonstrates strong interpersonal, organizational, and time management skills.
- Excellent communication skills both written and verbal are necessary.
- Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents if necessary.
- With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessary.
- Must demonstrate ongoing participation in educational opportunities.
- Must have the ability to travel.

M – Medium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. $67,500.00 – $90,000.00

14607 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
