Full Time Job Summary:

The Care Coordination Intake Specialist is responsible for ensuring the accurate completion, documentation, and distribution of all activities related to intake, authorizations, and re-authorizations of client services within established timeframes for Growing Home Southeast. This position requires collaboration with organizational leadership and departments to effectively and efficiently meet the organization’s needs. The Coordinator will continually assess the effectiveness of organizational processes and suggest service improvement changes. Additionally, the Coordinator will be responsible for reviewing authorization documentation, maintaining a schedule of re-authorizations, and troubleshooting concerns related to utilization with agency leadership and program staff.

Responsibilities:

Accept and coordinate all referrals for South Carolina programs, where applicable, ensuring the receipt of necessary demographic and insurance information, clinical history and other relevant information from referral sources that will be essential to placement and determining service needs. Facilitate the placement and services process for referrals in the areas of Therapeutic Foster Care (TFC) and Rehabilitative Behavioral Health Services (RBHS) in South Carolina. Communicate placement information to all necessary internal and external staff, while ensuring intake and placement requirements are met within the associated time frames. Create and maintain tracking systems organization’s database for referrals for applicable programs to include information on referral sources and service areas, while generating weekly reports to assist regional and corporate staff with placement data. Maintain current and accurate foster home information in our database at all times to assist in placement process Work closely with relevant internal and external staff to ensure compliance with all intake/placement requirements and associated time frames Maintain a schedule of re-authorizations to ensure timely submission and minimize service interruptions. Assist in troubleshooting concerns related to utilization, including overutilization or underutilization of services, and collaborate with agency leadership and program staff to address and resolve such concerns. Assist with other duties as assigned.

Qualifications:

Bachelor’s degree in a relevant field (e.g., psychology, social work, healthcare administration) or equivalent experience. Previous experience in intake coordination, authorizations, or a related field within a behavioral health organization is preferred. Knowledge of foster care and rehabilitative behavioral health services processes and regulations in South Carolina is highly desirable. Strong organizational skills with attention to detail and ability to prioritize tasks effectively. Excellent verbal and written communication skills, with the ability to collaborate and communicate effectively with internal and external stakeholders. Proficiency in using computer systems and databases for data entry and tracking purposes. Ability to review authorization documentation, identify discrepancies, and resolve issues with agency leadership and program staff. Experience maintaining a schedule of re-authorizations and addressing utilization concerns. Ability to work independently and within a team, demonstrating a proactive and problem-solving attitude. Flexibility to adapt to changing organizational needs and priorities. Commitment to maintaining confidentiality and adhering to ethical standards in handling sensitive client information.
