Job Duties:

Serves as a direct connection for former foster youth to reconnect with DSS and its services, such as Chafee/ETV benefits, Extension of Foster Care, Providers and contract holders serving this population, other government agencies and its benefits to secure stability and transitional growth. Provide readiness workshops, trainings, and events to support successful transition to adulthood. Engages and provide ongoing assistance, training and transition support to young adults that are receiving After-Care Services for former foster youth. Maintains minimum of monthly contact (virtual, email, text, Face-time) and adjust accordingly based on the needs of the young adult. Document all interactions in CAPSS. Completes, reviews, and updates individualized and strength-based transition plan in partnership with the young adult in a developmentally appropriate and identity affirming ways. Provides Service Navigation by identifying community resources, assist with referral and/or application processes, obtaining timely, essential, and appropriate support resources based on the young adult’s needs and goals. Follow up with received services, and ensure connection is solidified to the support resources. Guide the young adult to navigate self sufficiency by encouraging to build lifelong connections to family and community. Submits all applications for Chafee/ETV funds for eligible former foster youth to ensure compliance with federal laws, state laws, SC regulations and agency policy. Enter approval letters as linked files into CAPSS. Monitor Chafee/ETV payment for timely delivery and trouble shoot payment errors. Document NYTD services for approved services once payment is rendered. Partners with state, regional, and local providers serving transition age youth to expand service array in areas of life skills development, youth advocacy, housing service array, financial literacy, employability, transportation ownership, education, etc. Creates networking opportunities with community partners to maximize transition supportive resources. Attends all required trainings assigned by HR, SDT, and supervisor, including adolescent training track, and webinars for further professional development of this role. Maintains an active routine of self-care and pursues support when necessary to minimize the threat of secondary trauma to physical and mental health. Performs other job related duties as requested or required, such as Shelter Duty. Minimum and Additional Requirements A Bachelor’s Degree and one (1) year of experience in Social Work, Behavioral Science, or Social Science.
