Patient Care & Navigation Specialist Job Summary The Patient Care & Navigation Specialist plays a dual role in ensuring an exceptional patient experience by combining front-end navigation with direct clinical support. This position supports patients through every phase of their visit – from intake and orientation to clinical care and follow-up – while assisting providers and maintaining clinical standards. General Responsibilities Patient Navigation & Administrative Support Welcome and orient patients upon arrival, explaining clinic flow and expectations. Conduct patient interviews to update medical records and gather visit-specific data. Assist patients with scheduling follow-up visits and accessing community services (e.g., PCP referrals). Maintain accurate documentation using electronic systems; monitor communications and route appropriately. Respond to non-clinical patient questions and provide guidance as needed. Foster a welcoming environment by maintaining tidy, safe spaces and alerting staff of any hazards. Clinical Support Perform preliminary testing and gather information for the provider Provide phlebotomy services and collect/process lab specimens per clinical protocols. Process treatment/prescription refill requests and support the documentation needs of providers. Follow and promote infection control protocols and universal precautions. Support providers with medical documentation, patient care tasks, and in-clinic operations. Help manage medical supply inventory and ensure exam rooms are properly stocked and prepped. Collaboration & Clinic Operations Collaborate with clinic leadership to support policy and procedure implementation. Assist in identifying staff or volunteer needs and help facilitate training. Maintain confidentiality and ensure compliance with HIPAA and clinic policies. Contribute to a culture of teamwork, empathy, and professionalism. Required Qualifications 2+ years of direct patient care experience. Working knowledge of infection control protocols and universal precautions. Experience with collecting, handling, and processing lab specimens. Understanding of and strict adherence to HIPAA compliance and patient privacy standards. Effective communication and interpersonal skills with patients, staff, and external partners. Strong organizational skills with the ability to manage clinical and administrative tasks. Ability to work independently and make sound decisions aligned with clinic standards. Comfortable working in a fast-paced medical setting.
