These clients are often in crisis and are seen by the team a total of 3x a week. The team will consist of a team leader, 2 licensed clinicians and 1 CSI. We see clients in Rockdale/Newton Counties
- Provide intensive in-home, school-based, and office-based Community Support Individual (skill based) services for the purpose of stabilizing the living arrangement, promoting reunification, and preventing hospitalization
- Services are delivered utilizing a team approach and are usually provided in the home and within the family system
- We do not serve clients with a primary diagnosis of Autism.
- Manage a full-time caseload (25-30 face-to-face counseling hours weekly
- Collaborate with children, families, schools, and other community members to build skills and achieve treatment goals
- Offer effective clinical services through all stages of treatment: Engagement, Empowerment, and Transition, per PTP protocols. Comply with all payor and PTP service documentation requirements using our state of the art, HIPAA-compliant, electronic medical record system (CareLogic)
- Attend supervision, clinical team meetings, and participate at regular agency trainings
Location:
Bachelor level or Master Level (unlicensed) to provide in-home services in Clarke, Oconee, and Walton Counties.
