Job Description

Paraprofessional Elite Care, Inc. Douglasville, GA 30133 The Company Serving the following counties : Fulton, Clayton, Rockdale, Cobb, DeKalb Elite Care, Inc. provides behavioral health services intended to improve family functioning by clinically stabilizing the living arrangement, promoting reunification or preventing the utilization of out of home therapeutic venues (i.e. psychiatric hospital, psychiatric residential treatment facilities, or residential treatment services) for the identified youth. Services are delivered utilizing a team approach and are provided primarily to youth in their living arrangement and within the family system. Services promote a family-based focus in order to: Defuse the current behavioral health crisis, evaluate its nature and intervene to reduce the likelihood of a recurrence; Ensure linkages to needed psychiatric, psychological, medical, nursing, educational, and other community resources, including appropriate aftercare upon discharge (i.e. medication, outpatient appointments, etc.); and Improve the individual child’s/adolescent’s ability to self-recognize and self-manage behavioral health issues, as well as the parents’/responsible caregivers’ capacity to care for their children.

Qualifications:

graduate of a college or university with a bachelor’s degree in a field other than human services with less than four years of full-time, post-bachelor’s degree accumulated

MH/DD/SA

experience with the population served, or a substance abuse professional with less than four years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling. Supervision shall be provided by a Qualified Professional with the population served until the individual meets one year of experience. The supervisor and the employee shall develop an individualized supervision plan upon hiring. The partied shall review the plan annually; or High School Diploma with a minimum of two years of Behavioral Health experience

Responsibilities Include:

Providing and coordinating behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members. Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Person Centered Plan. Working as a team delivering services primarily to children in their family’s home with a family focus to: Defusing the current crisis, evaluate its nature, and intervene to reduce the likelihood of a recurrence. Ensure linkage to needed community services and resources. Provide self-help and living skills training for youth. Provide parenting skills training to help the family build skills for coping with the youth’s disorder. Monitor and manage the presenting psychiatric and/or addiction symptoms Work with caregivers in the implementation of home-based behavioral supports. Direct intervention and also arranges, coordinates, and monitors services on behalf of the recipient. Shall provide this service in any location. Keep the lines of communication open. Assists with maintaining good standards of living and quality care of the clients. Substantiate required service objective documentation. Not responsible for transportation. All staff has the responsibility for the following under the direction of the team leader: Participating in the person-centered planning process. Assisting with implementing a home-based behavioral support plan with the youth and his or her caregivers as outlined in the Person Centered Plan. Providing psychoeducation as indicated in the Person Centered Plan. Assisting the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress towards goals identified in the Person Centered Plan. Assisting with crisis intervention. Assisting the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process. Will receive supervision by the team leader.
