COMMUNITY HEALTH WORKER PROGRAM

ASSISTANTSalary – $51,815.00 per yearThis is a full-time, permanent appointment contingent upon Civil Service examination.

The Public Health Community Health Worker is responsible for building a positive awareness of the Public Health Department and their services within the community. The incumbent acts as a liaison to community partners and collaborating clinical and social service partners. This position has the responsibility for performing a variety of day-to-day administrative activities and tasks in support of the Public Health Department. The incumbent provides information and assistance at public education outreach and health screening events, conducts health presentations at community agencies, and provides support as needed to all public health branches. The duties are performed under the direct supervision of the Deputy Director of Public Health and/or Registered Professional Nurses. Some leeway is allowed for the use of independent judgment following established agency policy. Does related work as required.

FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS

Thorough knowledge and understanding of the nature and scope of services available through the Public Health Department; Good knowledge of modern public health practices and principles; Good knowledge of public information practices and public relations techniques; Good knowledge of community organizations and service providers related to public health; Ability to communicate effectively both verbally and in writing; Ability to understand and interpret complex oral instructions and/or written directions; Ability to work effectively with employees, members of the public, and staff of external agencies; Good organizational and public relations skills; Good judgment, integrity, tact and courtesy; Physical demands commensurate with the demands of the position.

MINIMUM QUALIFICATIONS

Graduation from high school or possession of a high school equivalency diploma and

EITHER:

A.Completion of 60 credit hours at a college or university accredited by a regional, national or specialized agency recognized as an accrediting agency by the U.S. Department of Education/US Secretary of Education and two (2) years entry-level clerical experience;ORB.Two (2) years of experience as a Licensed Practical Nurse, Community Health Worker Program Assistant, or Medical Assistant; ORC.Four (4) years senior-level clerical experience; ORD.An equivalent combination of training and experience as defined by the limits of A and B above.

SPECIAL REQUIREMENTS

1.Possession of a valid New York State driver’s license is required at the time of appointment and must be maintained throughout employment.2.Must be comfortable operating and driving the Public Health Mobile Sprinter Van.
