Summary:

The Care Coordination Program Assistant for Prime Care Coordination (PCC) is responsible monitoring and tracking all care coordination related administrative duties and documentation monitoring for the region assigned. Under the direction of the Regional Director and conjunction with the Care Coordination Program Specialist, the Care Coordination Program Assistant is responsible for collecting and tracking all required documentation, upload documents into the Electronic Medical Records(EMR) system and sent to Headquarters as needed.

Essential Job Functions:

Uploads all required documents including but not limited to: enrollment documents, disenrollment and annual review paperwork into PCC’s EMR and sent to headquarters as required. Responsible for tracking and reporting on all required care coordination documentation within assigned region Works directly with the Care Coordination Program Specialist on monitoring all required documentation for Care Coordination. Data monitoring and entry into MEDISKED and other PCC systems. Serves as the main contact for the region all CAS assessment communication to Care Managers from the Care Coordination Program Specialist which includes but not limited to: monitoring status of completion and reporting with to Care Coordination Program Specialist Schedules meetings as needed for Regional Directors, including coordinating use of conference and/or meeting areas. Takes meeting minutes as needed and distributes to attendees in a timely manner. Responsible for maintaining regional Care Coordination staff roster and contact information Ensure completion of all documents filing pertinent to department’s needs including but not limited to documents needed for audits Develops and ensures consistency for regional correspondences as needed to support the Care Coordination team e.g. create, copy, and distribute letters, memos, reports, and forms on a timely basis. Maintain the highest level of confidentiality, following all HIPAA policy and procedures. Meet professional obligations through efficient work habits such as meeting deadlines, honoring schedules, coordinating resources and meetings in an effective and timely manner. Perform all other duties relevant to the position as requested. Knowledge, Skills, and Abilities Proficient in the use of all Microsoft applications (excel, work, power point, Microsoft outlook and access). Ability to work independently under minimal supervision. Ability to communicate effectively, both orally and in writing. Ability to be efficient and productive in a fast-paced environment. Must be detail oriented and have the ability to multi-task.

Education and Experience:

Associates Degree preferred, HS/GED diploma. Two year’s experience working with and/or supporting

OPWDD/DOH

programs.
