HYBRID POSITION WORK FROM HOME SEVERAL DAYS A WEEK
AdvanceCare Health Services, LLC is seeking a compassionate and dedicated Program Assistant to train for the position of Program Manager within our DDA Department. This role is essential for ensuring the well-being and independence of the individuals we serve by overseeing their care and maintaining compliance with state and agency requirements. This is a hybrid role, combining work-from-home flexibility with on-site responsibilities at our Hermitage, TN office as needed. You will need to have a 27″ or larger monitor, keyboard, and mouse to use with our company-issued laptop. (If you are coming to the office to pick up your equipment, we can loan you the monitors, keyboard, and mouse as well). You must have high-speed internet.
JOB DUTIES
Ensure visits, staffing and other plans are kept up to date Ensure 24-hour visits are completed per policy Ensures staffing plan is complete Ensures Individual Support Plan is complete Obtain and upload documentation and other items as required to maintain up-to-date records Facilitates monthly PCSP and Individual Support Plan reviews Cooperate with Incident/Reportable Event Investigations Perform site visits and audits on schedule Provide Training including but not limited to pre-service and in-service training. Work with families, providers, and other agencies monthly Assist as needed with Incident Management and Corrective Action Committees Enforce standard company policies and procedures and ensure all are in compliance. Develop schedules for annual drills, appointments, meetings, including evacuation drills, medical appointments, reports, and provide reports to the director. Monitor and follow up on required drills and community visits as required. Ensure all client files, records, plans, and lists are up-to-date. Maintained according to policy/ procedure. Meet the attendance requirements established by the Director. Updates client emergency contact lists, client personal records, health care plans, and program staff contact lists. Performs other duties as assigned and needed.
SKILLS AND QUALIFICATIONS
Ability to work cooperatively and courteously with others. Ability to manage multiple tasks and projects. Self-directed with the ability to work with little supervision. Accepts performance-based criticism and direction. Thoroughness and careful attention to detail. Ability to document and analyze documentation Ability to think and operate independently with minimal supervision. Ability to handle all situations with professionalism and a positive attitude and effectively communicate with individuals at all levels of the organization. Knowledge of office computer applications and Microsoft Office suite of applications (e.g., Word, Excel, PowerPoint) and/or business management software.
REQUIREMENTS
High school diploma or GED equivalent; caregiving and office experience preferred. Previous experience in healthcare or a corporate environment is a plus. Satisfactory criminal background check. Flexibility to adapt and learn in a fast-paced environment.
Work Environment:
This hybrid position allows for several work-from-home days per week, with in-office attendance as departmental needs require. You must be flexible and willing to work and learn on the fly!
