Facilities Lead Technician

Program Manager – Social Care Network

ROLE DESCRIPTION

This role will oversee Home Accessibility and Safety Modifications, Mold and Pest Remediation, and Ventilation System Improvements to ensure Medicaid members’ safe and healthy living conditions. The ideal candidate will have a strong background in dwelling assessments, quality management systems, and compliance documentation. They will work closely with Social Care Navigators to document housing conditions, recommend remediation services, and oversee project execution while ensuring health and safety regulations adherence.

RESPONSIBILITIES

Project Management & Execution Oversee home accessibility modifications, mold and pest remediation, and ventilation system improvements (air conditioners, heaters, humidifiers, dehumidifiers). Supervise contractors, vendors, and maintenance teams to ensure timely, high-quality work completion. Develop and implement work schedules, budgets, and project plans, ensuring compliance with health, safety, and Medicaid regulations. Ensure that all modifications align with Medicaid guidelines and enhance the safety and well-being of residents. Dwelling Assessment & Compliance Conduct comprehensive assessments of Medicaid Members’ primary dwellings to identify environmental health risks. Prepare Statements of Work (SOWs) for approval by the Social Care Navigator before project execution. Ensure all medical necessity documentation from providers is included in compliance reports. Quality Control & Reporting Implement and maintain a Quality Management System (QMS) to assess and verify the quality of completed work. Conduct post-project inspections and evaluations to ensure modifications meet safety and compliance standards. Maintain detailed documentation of project assessments, remediation plans, and completion reports. Collaboration & Stakeholder Communication Coordinate with Social Care Navigators, healthcare providers, and contractors to facilitate service delivery. Communicate project timelines, challenges, and resolutions to stakeholders. Provide training and guidance on home modification best practices to team members and subcontractors. Qualifications & Skills Strong knowledge of housing health hazards, building codes, and remediation best practices. Experience with dwelling assessments and environmental health risk evaluations. Proficiency in project management software, budget tracking, and compliance reporting. Ability to work with vulnerable populations and understand the impact of housing conditions on health. Excellent organizational, communication, and leadership skills. Knowledge of Medicaid program requirements and home care regulations is a plus. Preferred certifications include Certified Indoor Environmentalist (CIE) or Mold Remediation Specialist (CMRS), and OSHA 30-Hour Certification (Construction or General Industry).
