Case Manager for Compliance and Data 5.0 5.0 out of 5 stars Buffalo, NY 14208 The Case Manager for Compliance & Data provides vital administrative and data support to the Say Yes school-based team, ensuring accurate management of confidential records, Medicaid compliance tracking, and timely data entry across multiple platforms. This role supports team coordination, family engagement, and contributes to organizational equity efforts. Additional supportive duties and tasks as needed to support the overall day-to-day operations of the School-Based Services department.

Major Responsibilities:

Management of secure electronic and hard case files, including oversight of Protected Health Information (PHI) Database search, oversight, and team communication regarding Medicaid compliance or lapses for all youth and children enrolled in Health Home Care Coordination. Data entry and uploads into multiple databases: inclusive of, but not limited to, Connections, Salesforce, NetSmart, and Microsoft Teams. Support with Family Satisfaction Survey completion and data input/process. Monthly Quality Checks and Audits for active and closed cases. Data processing and assessment for program development and modification. Secondary Responsibilities Preparation for the team meeting space, scheduling, and agenda. Family support and direction within the school-based team. Small projects support and organization. Some job duties described above can be completed remotely, while others will require office materials/space.

Minimum Requirements:

Data Entry experience Experience with Word, Teams, PowerPoint Point and Google Docs Ability to work with confidential data Must be able to remain stationary (at the workstation) for the majority of their work shift Believe and practice
