The Manager’s principal focus is GTF’s Direct Financial Assistance Programs in addition to serving as support in other patient service areas as identified by the Director of Patient Services. Minimum Qualifications Master’s degree in social work from an accredited school of social work. A minimum of two years work experience in transplantation or related medical social work field is required; five years preferred. Demonstrated success in program implementation and delivery. Must have a good understanding of solid organ transplantation. Must have a good understanding of transplant and health care issues related to patients and their families. Demonstrates knowledge of community resources for the patients, their families and the communities served. Practices confidentiality in accordance with standard social work practices. Must have effective client intervention skills. Must be able to work effectively under deadlines, with multiple tasks and with individuals representing all levels within the organization. Strong oral and written communication and presentation skills. Essential Duties and Responsibilities Oversee the Direct Financial Assistance Program Manage the application process Manage the tracking system for program usage and statistics In-service and educate all transplant center social workers about the program parameters and guidelines in conjunction with Director of Patient Services In-service all new transplant center social workers about the program and its application process in conjunction with Director of Patient Services Work closely with the Patient Service Advisory Committee for program development and management. Work with the Director of Patient Services to monitor the program’s financial aspects, including the entire program’s budget and/or aspects usage of the program funds in accordance to grant funding. Process medication and lodging billing in a timely manner on a monthly basis. Provide case studies for use in GTF publications, Board reporting, etc., in compliance with HIPAA guidelines. Participate in internal and/or external committees and/or groups, as requested. Participate in education and outreach efforts, as requested. Provide telephone support to clients served. Assist with GTF’s outreach initiatives for patient program awareness.
