Social Worker/Financial Empowerment Care Manager Jewish Family and Children’s Service of Greater Philadelphia – 3.9 Bala-Cynwyd, PA Job Details Full-time $55,000 – $60,000 a year 18 hours ago Qualifications Master of Social Work Mid-level Microsoft Office Driver’s License EMR systems Social Work Care plans 2 years Communication skills Full Job Description Summary The Social Worker/Financial Empowerment Care Manager (FECM) position offers a unique opportunity for an individual with work in a growing initiative at JFCS as part of the Financial Empowerment Program. Through financial coaching, the individual will work with low-to-moderate income individuals/families to identify financial goals and develop necessary steps to achieve goals. With the overarching goal of increasing financial stability, the FECM will work with clients to build assets, reduce debts, and make positive behavior changes. The FECM will be responsible for facilitating and creating Financial Capability workshops and programs, training and education, and other related financial literacy tasks. The Social Worker/FECM also works with an integrated team approach in providing care management services and supportive clinical counseling for Individual and Family Services (IFS) clients. The SW/FECM identifies needs of clients and provides care management services (resource navigation, benefit application/enrollment, and financial stability improvement). Services are provided at all JFCS locations and will serve the Greater Philadelphia Area, including remote work. The SW/FECM will provide services in client homes, office (primarily at our Northeast Philadelphia collaborative site), and remote settings; travel throughout the Greater Philadelphia area is required. The split in the role is approximately 60% FICM and 40% IFS Social Worker. Duties and Responsibilities FECM Participate in trainings that enhance knowledge in credit counseling, and financial capability. Conduct one-on-one coaching sessions with participants in JFCS’ Financial Empowerment Program. Assist clients to identify financial goals (e.g. budgeting, improving credit, increased financial stability, establish/increase savings) and develop steps towards goals. Monitor and support client progress towards goal achievement. Provide training, information, and resources to team members. Facilitate Financial Capability workshops and trainings to small community groups, agency clients, and partner organizations Works as a team with other JFCS staff with shared clients, resources, needs, and other related topics.? Social Worker Coordinate and provide supportive counseling and care management services for IFS clients through direct services in the client’s home, via telehealth, and in the office. Home visits are made to prevent access related issues for the client and made at their request. Conduct comprehensive assessments of clients’ needs and assesses their goals to develop mutually agreed upon treatment/care plans. Work with clients to enable access to resources appropriate to their individual needs (e.g., budgeting, financial issues, and accessing public entitlements). Collaborate with community resources (career services, housing) and assists clients in daily life skills to help them function at as high a level as possible. Maintain knowledge of a broad variety of community resources and refer clients when appropriate to support?their treatment goals. Provide advocacy with landlords, community enforcement agencies, family and natural supports, and others as needed.? Prepare and document accurate and timely records including progress notes, assessment tools, and billing information in compliance with department documentation guidelines and expectations and external regulatory policies. Participate in team conferences, staff trainings, weekly supervision, and other meetings related to care management issues. Provide two days a week of in-office service to clients who need brief support including benefits outreach, address walk-in needs, and other related tasks in our Northeast Philadelphia collaborative space.? Ability to recognize implied problems underlying initial statements of need and to ask questions appropriate to gathering more information to better serve the client and the agency. Must be able to maintain confidentiality. Ability to participate as a contributing team member of the department. Ability to work as a team member with other clinicians and care managers to ensure best service to clients. Ability to establish good working relations with other professional both internally and in the community. Flexibility and the ability to adapt to changes in schedule and assignments. Demonstrates initiative in contributing to the development of the department’s programs and services as well as his/her own professional development. Must be in good standing, maintain any pertinent license(s), and follow guidelines as required by the PA State Board of Social Workers, Marriage and Family Counselors, and Professional Counselors. Demonstrate knowledge, understanding, and implementation of practices based on the NASW code of ethics. Requirements MSW required Two to three years direct service experience required Knowledge of and experience applying for Public Benefits (SNAP, LIHEAP, SSI, etc.) a plus Proficient user of technology (smart phone/tablet/laptop) and experience using MS Office applications required EHR/EMR database experience a plus Additional Requirements Employee must meet agency vaccination requirements at hire and ongoing as required by policy. All employees must complete background checks required in keeping with individual departmental and agency requirements. May host groups that run beyond normal business hours (schedule will be adjusted accordingly) Travel may be required within the Greater Philadelphia Area based on program needs. Access to a car with valid driver’s license or ability to use public transportation. The position may work at several office locations including Bala Cynwyd and North Philadelphia. Remote work may be available as agreed upon with associate’s leader. Employee must have a safe driving record on file with the Department of Motor Vehicles in the state in which the employee is licensed to drive. Employee must provide a valid driver’s license and proof of automobile liability insurance. In addition, employee must provide security clearances free of any citations.
