Coordination of employee schedules as it relates to theApproved Care Schedule and Plan of Care. Processing and maintainingpaperwork (paper and computer based) in accordance with State /Federal laws and in compliance with Action Health Staffingpolicies. Provide direct care to clients when needed or requested.

Greet visitors, receive incoming telephone calls and faxtransmissions. Transfer calls and faxes appropriately. Maintain awelcoming and clean environment.

Reports to:

Directly – CorporateSupport Specialist Indirectly – Registered Nurse / Sr.

VicePresident Supervises:

HHA’s, PCA’s, Homemakers, Field Staff

QUALIFICATIONS

Current or Past CNA/HHA certification preferred orexperience working in a health care environment. Time management,organizational skills, and ability to function with minimaldirection.

Experience working with staffing and schedulingpreferred; preferably within a health care setting.

Currentknowledge or ability to learn and comply with all state and federalemployment laws and AHS policies.

Effective communication skills(verbal and written), documentation skills, interpersonal skills,and problem-solving skills.

Ability to be trained and comply withall regulatory requirements for personnel in home healthcare.

Excellent telephone, communication, interpersonal, anddocumentation skills.

Computer literate and ability to workeffectively with companies computer programs.

ESSENTIALFUNCTIONS/AREAS OF

ACCOUNTABILITYGreet all visitors and answerincoming calls in a pleasant and proper manner with a welcomingdemeanor and direct them to the appropriate person ordepartment.

Maintain confidentiality and assure documents aresecured in appropriate areas / comply with HIPAA.Maintain aprofessional demeanor at all times. Do not partake or initiateinappropriate conversations / be a team player.

Personal calls andpersonal texting need to occur only in an emergency situation whilein the office.

Assure that cases are staffed with appropriatelycredentialed staff and within established time frames.

Completedocumentation timely (paper and automated system).Provideappropriate notification of schedules to employee, client, oroutside business.

Monitor overtime which must have prior approvalfrom supervisor. Rotate with other HSM’s being on call duringnon-office hours and handle those functions effectively. Personallyfulfill in-home staffing needs as required or requested. Renderhands on care. Verify the required staffing services are renderedeach day/week according to the Plan of Care/ACS.Assure thecompletion of all clerical functions such as the proper filing oftimesheets, data entry, etc.

Assist in the review timesheets toensure accurate services were provided.

Familiarize self to clientand staff charts so documentation can be located upon request.

Plan,direct, and participate in recruitment and retention functions.

Accept and review applications, run proper checks, interviewapplicants, and input into the system.

Provides direction to fieldstaff and communicates with supervisors as needed to assure safeand effective coverage of client needs.

Assign fully screened agencystaff to clients as directed by the care plan and servicerequest.

Assure employees receive complete and accurate directionsand information about client needs.

Notify client of staffingchanges and monitor employee performance in areas of dependability,responsiveness, timeliness, and client concerns. Respect PatientBill of Rights.

Carry out the evaluation and performance review offield staff. Utilize Call Logs daily and when on call and maintainaccording to policy.

Follow up with employee on schedulingproblems.

Determine hiring needs and follow through with supervisorregarding any staffing shortages.

Minimize substitution of employeeto maximize continuity of care.

Maintain adequate numbers ofavailable on-call staff.

Keep nursing staff apprised of any changesin clients conditions / needs / or request.

Provide consultation anddisciplinary action paired with required documentation whenwarranted.

Monitor and assist in managing all in-service needs andrequirements for staff.

Responsible to alert staff regarding theneed for any and all personnel file items. Maintain all timesensitive information in an up to date form and information kept inemployee files and entered into the computer system. Maintain theOn Call book with complete and accurate information.

Maintainconfidentiality and safeguard all patient information. Respect theconfidentiality of information in both client and employeerecords.

Share information in accordance with agencypolicies.

Perform duties in compliance with state / federal laws andin accordance with AHS policies and procedures. Attend meetings andeducational programs as required.

Maintain a professional stancewithin the community and throughout social network platforms.

Participate in the agency’s ongoing quality improvementactivities.

Review client service hours weekly and follow-up withstaff, nurse and client.

Perform other related job duties andresponsibilities as assigned by your supervisor.

Run all monthlyrequired reports and log / file information.
