Social Companion Associated Care Ventures Inc Montgomery, AL Associated Care Ventures Overview Associated Care Ventures (ACV) is the Birmingham-based parent company of Simpra Advantage, Inc., an Alabama domiciled insurance company operating an Institutional Special Needs Plan, a Dual Eligible Special Needs Plan, and an Institutional Equivalent Special Needs Plan. ACV also is the parent company of Alabama Select Network, LLC, which is operating an Integrated Care Network for Alabama Medicaid, to coordinate Medicare and Medicaid services state-wide for eligible individuals receiving long term services and supports, both living at home and in nursing facilities. Provide companionship services for eligible Simpra members residing in long term care facilities and at home as directed by the Social Companion Coordinator • In the institutional Special Needs Plan (Nursing Home) Many members have limited social engagement and just want someone to talk to Possible activities include: read to member, listen to music, story telling Painting their nails Helping with nursing home-led activities such as bingo Puzzles, crosswords, board games Arts and crafts • In the Dual Eligible Special Needs Plan (Members’ Homes) Companionship Transportation (within 25 miles of member’s home which is incidental to offering social companionship, such as shopping, running errands, picking up prescription medications, or a doctor visit) House tasks/ Assistance Technology Education and Support Physical Activity/ Exercise Grocery Shopping Care Gap Reminders as directed by Simpra RN Care Coordinator Identify and report on social determinants of health concerns, unmet needs to Simpra RN Care Coordinator Pet Help/ Assistance Position Qualifications Valid Driver’s License/ Vehicle/ Proof of Automobile Insurance coverage with liability limits of at least $100,000 per person and $100,000 per accident Must be able to travel to Birmingham office on first day for onboarding Position Skill Requirements Skill Personable, motivated, energetic, reliable Basic computer skills such as email, electronic forms Ability to work with older population Positive communication skills Compensation $ 20/ hour; up to 30 hours per week as needed and available + mileage reimbursement
