Position Overview:

We are seeking dedicated and experienced Long-Term Services and Supports (LTSS) Service Care Managers to support Medicaid members with complex needs. This field-based position will work closely with elderly populations (65+) and their caregivers to coordinate care, conduct assessments, and ensure quality service delivery.

The role involves 80-90% travel for in-person visits within a designated Florida region. The ideal candidate is independent, proactive, highly organized, and passionate about supporting vulnerable populations.

Key Responsibilities:

Conduct in-home or on-site assessments of members’ health, social, and support needs. Develop, coordinate, and monitor personalized care/service plans for long-term care members. Engage and educate members and caregivers on healthcare benefits and available services. Ensure provider services are delivered without gaps and monitor care delivery. Coordinate with physicians, discharge planners, and community partners for smooth transitions and resource connection. Maintain accurate documentation of all case management activities within 24 hours of completion. Assist members with complaint or appeal processes when needed. Track outcomes and suggest improvements to service delivery models.

Must-Have Qualifications:

Bachelor’s degree in a healthcare-related field (e.g., Nursing, Psychology, Sociology, Social Work). 2+ years of field-based Care Management experience, especially in long-term care Medicaid/Medicare populations. Experience managing high caseloads (typically 50-70 members) and documenting care within EMR systems. Strong knowledge of Medicaid, Medicare, and HCBS (Home and Community-Based Services). Home health experience and familiarity with service authorization and coordination.

Tech-savvy:

comfortable troubleshooting equipment in the field and coordinating with IT. Valid Florida driver’s license and reliable transportation. Typing test required•results must be included with application. Preferred (Nice to Have): Experience with discharge planning or transitional care coordination. Bilingual (English/Spanish) is highly preferred. Prior experience with electronic medical records (EMR) systems.

Disqualifiers:

No field-based care management experience. No experience with high caseloads or elderly population. Candidates located outside of approved Florida regions (ZIP code required on resume). Top 3
