Clinician 2.3 2.3 out of 5 stars 1700 Education Avenue, Punta Gorda, FL 33950 Provides clinical assessment, service coordination, therapeutic interventions, and discharge planning for the residential program. Assessment of individuals with substance use disorders with treatment recommendations provided to treatment team and other service providers. Collaborates with internal and external treatment providers to ensure appropriate treatment recommendations and discharges plans are in place. Conducting individual and family sessions, group therapeutic activities, and discharge planning, as needed, for adults who present for service. Responsible for accurate and timely documentation of all information in the clinical record. Actively respond to individuals who show signs of crisis, which may include mental health symptomology or unsafe behaviors. Work as a team with other staff to de-escalate. Assists with screening referrals for appropriateness and coordinating admissions Communicate admission to agency staff, programs and / or community partners who are currently involved with patient. Develop a behavioral treatment plan with measurable objectives that the individual can work toward while completing residential treatment. Complete chart audits while updating and monitoring accuracy of clinician tracker. Bachelor’s degree or higher in human services field. Minimum one (1) year experience working with individuals in behavioral health. Ability to pass a level II Background clearance and drug test. Valid FL drivers’ license, insurance, and safe driving record. Ability to lift and move objects weighing up to 25 pounds Stamina to stand, walk, sit, and perform tasks for extended periods.
