Role:

We are seeking a highly organized and compassionate LTSS Service Coordinator to provide case management and facilitate Person-Centered Planning for individuals in specialized programs. This role ensures holistic care coordination by integrating behavioral and physical health services while empowering individuals to achieve their goals. This job will have the following responsibilities: Conduct face-to-face program assessments using standardized tools and motivational interviewing techniques. Develop and maintain a comprehensive Person-Centered Support Plan (PCSP) aligned with member preferences and care needs. Serve as the primary point of contact for members, coordinating health services and long-term support. Engage with Medical Directors and participate in interdisciplinary care rounds to support integrated care planning. Facilitate care team collaboration , including caregivers, family, service providers, and physicians. Identify alternative service options or waiver programs for qualifying members. Submit utilization and authorization requests , ensuring compliance with care plans. Document and report critical incidents to state and county agencies as required. Assist in grievances, appeals, and state audits . Mentor and train new staff, supporting process improvement initiatives .

Qualifications & Requirements:

Bachelor’s degree in healthcare or social work (preferred) OR equivalent combination of education and experience. Minimum of 2 years of experience in a social work agency or related field. 701 Certificate & CPR Certification (Training available upon onboarding if needed). Bilingual proficiency (Preferred). Strong time management, organizational, and interpersonal skills .
