Job Discerption:
Position Purpose:
Supports care management activities and assigned teams to ensure member services are efficiently delivered by healthcare providers and partners. The role helps ensure continuity of care and member satisfaction through proactive engagement, outreach, and coordination.
Key Responsibilities:
Conduct outreach to members via phone (or home visits as necessary) to review care/service plans, answer questions, provide education, and ensure follow-up. Coordinate care activities across healthcare and community resources per care/service plan. Address member and provider inquiries, issues, and changes in care plans. Maintain communication with care managers, practitioners, and support teams to facilitate seamless care. Perform service assessments and screenings when applicable and document member needs. Maintain accurate records in compliance with regulatory and internal standards. Identify and refer members to appropriate resources such as Care Managers, community-based organizations, or Disease Management programs. Educate members on available benefits and health resources. Submit daily production reports and adhere to assigned call metrics (50-75 calls/day). Be prepared to work in-office if remote access is disrupted (e.g., due to power/internet outages). Perform other duties as assigned.
Education & Experience:
Required:
High School diploma or GED 1-2 years of related experience
Preferred:
(Spanish or Creole) Previous remote work experience
Technical Skills & Tools:
Proficient in Microsoft Office (Excel, Outlook, Word) and Microsoft Teams Comfortable with using internal care systems and performing online research Ability to multitask: handle calls while documenting information
