Full-Time, Standard Business Hours typical hours are 8-5 Monday to Friday, may be 7:30-4:30 instead.

Position Overview:
As a Long-Term Care Coordinator , you will develop, assess, and coordinate holistic care management plans for members 65 and older. You’ll support high-quality, cost-effective care by aligning services, coordinating with healthcare teams, and empowering members and their caregivers.
Key Responsibilities:
Evaluate member needs and available resources to guide effective care Create and monitor ongoing long-term care service plans Coordinate care between members, families, providers, and community resources Provide referrals for services including housing, employment, and independent living Conduct in-person visits to assess member needs Educate members and caregivers on services, healthcare instructions, and benefits Maintain accurate documentation and regulatory compliance Offer recommendations to improve care quality and service delivery
Qualifications:
Minimum 2 years of field-based case management experience Experience working with older adults (65+), especially in long-term care settings Strong communication and time management skills Comfortable with technology and documentation systems Typing test completion is required Make a real impact in the lives of older adults—apply today!
