Job Summary:

We are seeking a dedicated LTSS Service Coordinator to conduct service coordination functions for a specialized caseload of individuals in our programs. This role involves collaborating with supported individuals to facilitate the Person-Centered Planning process, documenting their preferences, needs, and self-identified goals. Key responsibilities include conducting assessments, developing comprehensive Person-Centered Support Plans (PCSPs) and backup plans, and managing the overall physical health (PH), behavioral health (BH), and LTSS needs of assigned members while meeting state, contractual, and federal requirements.

Primary Responsibilities:

Conduct face-to-face assessments using standardized tools to identify and evaluate individuals’ waiver (LTSS/IDD), behavioral health (BH), or physical health (PH) needs, applying motivational interviewing techniques. Facilitate the Person-Centered Planning process to develop fully integrated care plans, engaging members and their circle of support, including caregivers, service providers, physicians, and natural supports. Serve as the single point of contact, coordinating care and addressing potential clinical health needs such as high-risk complications or gaps in care. Manage non-clinical needs of members with chronic illnesses, co-morbidities, or disabilities to ensure efficient utilization of long-term services and supports. Submit utilization and authorization requests to utilization management teams aligned with individuals’ care plans. Report critical incidents to appropriate internal and external parties, including Adult Protective Services and law enforcement. Participate in appeals, grievances, hearings, and state audits as needed. Identify alternative service levels or waiver programs to better address members’ needs. Provide mentorship, training, and support to new staff, serving as a subject matter expert or preceptor as required. Contribute to process improvement initiatives.

Qualifications:

Bachelor’s degree (BA/BS) in social work, healthcare, or a related field (preferred). Minimum of 2 years of experience working within a social work or healthcare agency, or an equivalent combination of education and experience.
