Aging Life Care Professionals consider each client’s unique needs, wishes, and beliefs based on initial and ongoing assessments to coordinate suitable third-party services and providers. They are responsible for guiding clients and their support systems in evaluating and understanding care options by conducting comprehensive assessments and facilitating the necessary services to achieve the client’s goals, interests, and expectations, ensuring positive outcomes. Candidates should be familiar with standard practices and procedures within senior care, enabling them to coordinate medical, legal, financial, household, and personal care services to support independence, safety, and quality of life. The ability to integrate the client, family, and providers to meet related needs and evaluate service outcomes is important. Field experience in aging care services is strongly preferred.
Key Responsibilities:
Advocate for the clients’ needs. Provide education to clients and their family members. Offer crisis intervention to assist families in understanding aging-related challenges. Enhance the client’s quality of life and independence through a person-centered care approach. Traveling within Lee County is required, with rotations on weekends and holidays.
Qualifications:
Bachelors or Master’s (preferred) Degree in social work, gerontology, nursing, psychology, physical therapy, occupational therapy, health care, health and human services, or a related field of human services delivery Current Florida State driver’s license and a registered, insured vehicle with the ability to move between locations Case Management or related certification a plus; assistance can be provided to obtain CMC certification and ALCA (Aging Life Care Association) membership In this position it is recommended to know about working with Guardianship, Conservatorship, Trusts, and related fiduciary issues. Excellent organizational and communication skills. High attention to detail and ability to handle sensitive information with confidentiality.
